Referral Process
Who can refer: Clients can self refer, GPs, specialists, SAH/NDIS support coordinators, and allied health professionals are all welcome to refer clients.

Urgency: Please indicate if the client requires priority scheduling.

Client details: Name, DOB, contact number, and Medicare number and expiryÂ

Current medications, pathology, and co-morbidities

Please attach CDM referrals
How to refer?
GP/CDM referrals: Please send via Medical Objects or HealthLink.
